I have a customer that wants to block all inbound messages from the internet except for a few specific addresses. Is there a way to do this on the server? Within the exchange environment is a breeze but from the outside???

One I've used: TrendMicro's Interscan eManager (might have a new moniker). Very easy to set up and good support. With this tool you might be able to show your customer how easily they can choose what to allow through by setting up various filtering rules (truly easy to do). You could also look at a firewall-based solution -- only allow SMTP traffic from specific IP addresses.
